Leadership Award In Honor of Jane Skrzypek

Purpose:
This NYS PTA Award was established in 1991 to honor
the memory of Jane Skrzypek. The purpose of this award is to recognize
an outstanding leader in your PTA who, by his or her actions, inspires
others to volunteer and assume leadership positions.
The award is given in recognition of outstanding PTA volunteerism - the
recipient must be a member of PTA, volunteer through PTA, volunteer
above and beyond the scope of his/her PTA position, and exhibit
leadership qualities.
NOTE:
You may wish to honor a volunteer who goes above
and beyond the scope of their position – a person you can always rely on
to be there when an extra hand is needed. You may honor a volunteer
within your unit by presenting them with a Jane Skrzypek Volunteer Pin
(available to purchase at the boutique at a cost of $5.00).
However, receiving the pin does not make the recipient a Jane Skrzypek
Volunteer of the Year Award winner. Only NYS PTA can select a winner. To
nominate a volunteer for the State Award, please read the following
instructions thoroughly.
Members of the New York State PTA Board of Managers are not eligible to
be nominated for this award.
All nominees receive a certificate of appreciation.
Eligibility:
To be eligible for nomination the nominee must:
Be a member of a PTA
Volunteer through the PTA
Volunteer above and beyond the scope of their PTA position
Exhibit leadership qualities
Application:
1. The form must be filled out
completely and signed by the president and one other officer of your
PTA.
2. Units, councils, or regions are eligible to
nominate volunteers for this award.
3. Each submitting group may nominate only one person
per year.
4. The short narrative on the form itself should be a
list of the nominee’s volunteer involvement.
5. Include, with the application, two statements of
why this person is deserving of this award. One statement should be from
an officer of your PTA. The other may be from the
principal/superintendent or from another PTA officer.
6. Be sure to cite how this person meets all the
criteria listed under the eligibility requirements. Each statement must
be submitted on one piece of 8 1/2” x 11” paper. No more than two (2
letters of commendation will be accepted.
7. The cover sheet of the application must include the
name, address, and phone number of a contact person who can be reached
for clarification if it is needed.
8. The application must be filled out and returned to
the PTA Region Director postmarked by June 15. The Region PTA may select
a winner to submit to New York State PTA for selection as a state
winner. (Applications will be forwarded to State by the July 15
deadline.) From the nominees submitted one recipient will be chosen by
the State Committee no later than September 1. The State winner will
be announced at the New York State PTA Convention in November.
